Axon Chiropractic & Rehab (“Axon,” “we,” “us,” or “our”), located at Suite 120, 200 Consumers Rd, North York, ON M2J 4R4, is committed to protecting the privacy and confidentiality of the personal information and personal health information entrusted to us by our patients and website visitors. This Privacy Policy explains how we collect, use, disclose, and protect that information, and how you can exercise your privacy rights.
This policy applies to information collected through our website (axonchiropractic.ca), in our clinic, over the phone, and through any other means by which you interact with us.
1. Who We Are
Axon Chiropractic & Rehab is a multidisciplinary clinic offering chiropractic care, registered massage therapy (RMT), and acupuncture services. As a health information custodian under Ontario’s Personal Health Information Protection Act, 2004 (“PHIPA”), we are legally responsible for the personal health information we collect and maintain, and we follow the standards set by the regulatory colleges governing our practitioners (including the College of Chiropractors of Ontario, the College of Massage Therapists of Ontario, and the College of Traditional Chinese Medicine Practitioners and Acupuncturists of Ontario).
Our practitioners include independent contractors and associates who provide care within our clinic; each is separately bound by their own professional and regulatory obligations regarding your health information.
2. Information We Collect
a) Personal Health Information (collected as a patient)
When you become a patient at Axon, we may collect:
- Contact and identification information (name, address, phone number, email, date of birth)
- Health history, symptoms, diagnoses, treatment notes, and progress records
- Insurance and billing information (extended health benefits, WSIB, auto insurance/MVA, etc.)
- Information shared during intake forms, assessments, and treatment sessions
- Referral and communication records with other health care providers, where applicable
b) Website and General Contact Information
When you visit our website or contact us online, we may collect:
- Information you voluntarily submit through contact forms, appointment requests, or email (name, email address, phone number, message content)
- Information submitted through our online booking platform, Jane App
c) Automatically Collected Information
Like most websites, our site may use cookies and similar technologies to collect:
- IP address, browser type, device type, and general location
- Pages visited, time spent on site, and referring website
- Analytics data through tools such as Google Analytics

- Provide chiropractic, massage therapy, and acupuncture care, and coordinate treatment among your care team at Axon

- Respond to inquiries submitted through our website
- Improve our website, services, and patient experience
- Meet legal, regulatory, and professional college record-keeping requirements
- Comply with applicable law, or respond to a valid legal request
We will only use your personal health information for the purposes for which it was collected, or for a directly related purpose, unless you consent otherwise or we are permitted or required by law to do so.
4. Consent
By becoming a patient at Axon, you consent to the collection, use, and disclosure of your personal health information as described in this policy and as needed to provide your care. You may withdraw or limit your consent at any time by notifying our Privacy Officer, subject to legal and professional obligations (for example, we are required to retain clinical records for a minimum period regardless of a consent withdrawal).
Submitting information through our website (e.g., a contact form) constitutes consent to be contacted by our clinic regarding your inquiry.
5. How We Share Your Information
We do not sell, rent, or trade your personal information. We may share information in the following circumstances:
- With your care team: Practitioners within Axon involved in your treatment, on a need-to-know basis
- With your consent: Referrals to other health care providers, or communication with your insurer, employer benefits plan, or legal representative, at your request
- Service providers: Third parties who support our operations, such as our booking/scheduling software Jane App, payment processors, and IT/website hosting providers, who are contractually bound to protect your information and use it only for the purposes we specify
- Legal requirements: Where required by law, court order, or to a regulatory college in the course of a professional conduct matter
- Billing purposes: Insurance companies or third-party payers, limited to what is necessary to process a claim
Any third-party service providers we use are selected with reasonable care and, where they process personal health information on our behalf, are required to maintain safeguards consistent with PHIPA.
6. Data Security
We maintain administrative, physical, and technical safeguards designed to protect your personal information and personal health information against unauthorized access, use, disclosure, alteration, or destruction. This includes secure storage of clinical records, restricted staff access on a need-to-know basis, and secure handling of electronic records.
No method of transmission or storage is 100% secure; while we take reasonable steps to protect your information, we cannot guarantee absolute security.
7. Retention of Records
We retain personal health information in accordance with the requirements of the applicable regulatory colleges and Ontario law. Generally, clinical records are retained for a minimum of ten (10) years from the date of the last entry (or, for a patient who was a minor at the time of treatment, ten years from the date they reached the age of majority). Website inquiry data not tied to a patient record is retained only as long as necessary to respond to the inquiry, unless you become a patient.
8. Your Rights
Under PHIPA, you have the right to:
- Access your personal health information, subject to limited exceptions permitted by law
- Request a correction to your personal health information if you believe it is inaccurate or incomplete
- Withdraw consent for certain uses or disclosures of your information, subject to legal and professional limitations
- File a complaint with our Privacy Officer, or with the Information and Privacy Commissioner of Ontario (IPC), if you believe your privacy rights have been violated
